A Clinical Evaluation of 197 Spinal Cord Injured Patients



Abstract
197 case of spinal cord injured patients admitted or visited to Veterans Hospital, ¢¥during the period from January, 1977 to January, 1982¢¥ were analyzed. The results were as follows;
1) The most common causes of the spinal cord injuries in the order of frequency were in traffic accident in 68 cases (34.5%) , gunshon in 50 cases (25.4%) ,_ falling down in 46 cases (23.4%o), electric accident in 13 cases (6.5%) , and the others were 20 cases (10.2%) .
2) Duration follwing injury were 11-15 years in 55 cases (28.0%) ; 0-5 years 39 cases (19. 81¡Æo); 6-10 years, 37 cases (18.8%) ; 16-20 years, 29 cases (14.7%) ; 21-25 years, 17 cases (8.6 %); 26-30 years, 14 cases(7.1%); 30 years(3.0¡Æ0).
3) The type of neuropathic bladder were revealed with 87 cases (44. 1%) in spastic type, 77 cases (39.1%) in flaccid type, 33 cases (16.8%) in uninhibited type.
4) The most frequent urinary drainage methods in spinal cord injured patients were urethral catheterization in 58 cases (29.4%), suprapubic cystostomy in 48 cases (24.4%), self Credes method in 48 cases (24.4%), condom drainage in 43 cases (21.8%)..
5) The most common urologic complication were 53 cases (26.9%) of urinary stone, 33 cases (16.8%) of pyelonephritis, 8 cases. (4.1%) of vesicoureteral reflux, 8 cases (4.1%) of urethral fistula, 8 cases (4. 1%) of vesical diverticulum, 5 cases (2.556) of hydronephosis 5 cases (2.5%) of epididymitis, 1 case (0.5%) of bladder tumor, and then the incidence of complication was increased in total flaccid type (88 cases: 44.7%) than spastic type (33 cases: 16.7%).
6) All of these patients had initial management by a careful- indwelling catheter, this method reduced the incidence of the vesico-ureteral reflux, but the incidence of bladder stone was increased in flaccid neurogenic bladder than spastic type.
